Consent Forms
All treatments require completion of a consent form. We will explain the treatment, aftercare and any risk to you thoroughly and ask that you sign the consent form prior to any treatment being carried out.
Emergency Appointments
We do operate a round the clock service. If patients require urgent care and the practice is closed, please email with your details and symptoms to manager@bespokedentalfulham.co.uk. Alternatively you can call the practice on 020 3375 6076 where you can leave a voicemail and we will endeavour to contact you as soon as possible to arrange an urgent appointment. Rest assured, voicemails left with the practice are monitored closely and a member of the team is able to contact you outside of practice hours.
Personal Details
It is important that you provide a full medical history and details of any medication you take or allergies you have upon your first visit to the practice. Should these change in any way you must inform the dentist and give up to date information.
General Data Protection Regulation
We store all patient personal details on a management system in accordance with the Data Protection Act. All clinical notes, digital radiographs, digital photographs etc remain the property of 76 Harley Street. Copies of notes, radiographs and photographs can be made available on request.
Please be aware that any information provided through any part of our website or on the phone or in person with anyone other than a dentist does not constitute professional advice. No professional advice can be given without a clinical consultation with a dentist.
Photography & X-Rays
76 Harley Street may use images/video and x-rays of your smile and teeth, for marketing and educational purposes on the practice website, and on promotional and educational literature. We will of course ask for your consent first before proceeding with photography or video. Your name will never be published, and identity will never be disclosed. However, if you DO NOT wish for us to use your images and x-rays in this way, please let us know.
Late Cancellation or Missed Appointments
If you need to cancel or change an appointment please give us as much notice as possible. The practice requires a minimum of 48 hours. Appointments cancelled without 48 hours notice will, at the practices discretion, incur a cancellation fee of £100 or more, dependant on the time allocated with the clinician. Failure to attend an appointment for treatment arranged over 2/hrs with the clinician, the practice has the right to charge 50% of the treatment fee.
For new patients arranging an initial consultation with any of our specialist dentists, failure to attend or provide 48 hours notice will result in your deposit being non-refundable. For active patients referred to any of our clinicians will incur a cancellation charge if our fail to attend / cancellation policy is not adhered to.
Please be aware that if two or more consecutive appointments are cancelled without 48 hours notice, or failed to attend (without good reason), we have a right to provide no further appointments at the practice.
No Tolerance/Abuse Policy
At 76 Harley Street we operate a zero tolerance policy to abuse to our Dentists and staff, loud/ disorderly/drunken behaviour, persistent missing and late cancellation of appointments. In these situations, we reserve the right to refuse treatment and admission.
